Prominent Ear Correction: A Comprehensive Review of Fascial Flaps in Otoplasty

A light bulb moment in the late 2000s during a tricky otoplasty operation resulted in me raising the postauricular fascia based distally (anteriorly) rather than proximally as previously described. It was immediately apparent that this versatile tissue could play a greater role in the development of otoplasty than simply covering suture knots. This prompted an investigation of the anatomy of this interesting structure, culminating in the concept of the intrinsic and extrinsic postauricular fascia and delineating its vascularity. A series of nuanced approaches to distally-based fascia otoplasty techniques were subsequently published over the next decade as reviewed by Tas, alongside all the otoplasty techniques that include fascia. Frascino first brought attention to the role of the perichondrium, which was further expanded by Cihandide et al some years later.
