Perforator based V-Y flaps: Versatility in upper lip reconstruction

Aims

The upper lip is a unique unit of the face which is formed of three subunits itself. Upper lip cancer is one of the commonest cancers plastic surgeon have to deal with. With the paucity of tissues in this anatomically special area, it is difficult some times to reconstruct like with like.

Method

Perforator based flap is a well-established reconstructive tool which has been practised over the last few decades in order to maximise the outcomes after excision of upper lip tumours.

Results

The reconstructed lips looked normal with minimal scarring. The height was preserved and the scars were hidden within the lines of the subunits. There was very minimal wound healing problems and the patients were very satisfied with the outcomes.

Conclusion

Perforator based V-Y upper lip reconstruction is a useful tool to minimise resultant scarring and maximise the pull of the flap. It is easy to perform with well predicted vascularity and rewarding outcomes.
