Ultrasound Guided Perineural and Extensor Carpi Ulnaris Tendon Sub-Sheath Hydrodissection in Management of Symptomatic Dorsal Cutaneous Branch of Ulnar Nerve Impingement

Ulnar-sided wrist pain is a common presenting clinical complaint and can present a diagnostic challenge for the treating physicians. Though individual pathologies of the dorsal cutaneous branch of ulnar nerve (DCBUN) and extensor carpi ulnaris (ECU) tenosynovitis can give rise to ulnar side wrist pain, we report an unusual distal impingement of the DCBUN secondary to ECU tenosynovitis as a presenting feature in a 35-year-old female posing a diagnostic dilemma. This case report highlights the importance of focused clinical examination, awareness of such a pathology, the role of dynamic ultrasound (US) in the diagnosis, and effective treatment of the patient's symptomatic DCBUN neuropathy with US-guided perineural and ECU tendon sub-sheath hydrodissection.
