Predictors of post-operative complications following immediate implant-based breast reconstruction - a single centre experience

Introduction
Post-operative complications are common in breast cancer patients who underwent immediate implant-based breast reconstruction following risk reduction mastectomy. This study aimed to determine patient and intraoperative factors that increased risk of post-operative complications.
Method(s)
161 patients underwent immediate implant-based breast reconstruction at North Manchester General Hospital Breast Surgery Unit between October 2018 and January 2023. Retrospectively, data was collected on patient factors [Age, body mass index (BMI), smoking status, American Association of Anaesthesiologists (ASA) grade], intraoperative factors (mastectomy type, mesh type, implant size, PICO dressing use and drain size) and post-operative complications were collected. Logistic regression was used to calculate odds ratio (OR) to determine risk factors. Statistical significance was p<0.05.
Result(s)
ASA grade 3 increased risk of return to theatre (RTT) within 6 months of initial operation [OR 5.25 (1.3-20.5), p= 0.01] and nipple areolar complex (NAC) necrosis [OR 9.18 (1.45-57.8), p= 0.02]. Body mass index (BMI) increased risk of seroma [OR 1.1 (1.01-1.19), p= 0.02]. Implant size increased risk of hematoma [OR 1.01 (1.003-1.03), p= 0.012]. Following multivariate analysis, ASA grade 3 increased risk of RTT within 6 months [OR 9.6 (1.9-48.1), p= 0.005] and NAC necrosis [OR 14.4 (1.3-149.1), p= 0.02]. BMI increased risk of seroma [OR 1.09 (1.003-1.19), p= 0.04]. Intraoperative factors did not increase risk of post-operative complications in our study.
Conclusion(s)
BMI and ASA grade of 3 are risk factors for post operative complications. Surgeons should take these factors into consideration when assessing eligibility for surgery and counselling patients on possible post-operative complications.
