Innovation in surgical technique is a cornerstone of advancing surgical care. The authors describe a new surgical technique that utilizes residual lobular tissue anterior and inferior to the ear to form a new earlobe.1 The authors believe that this approach brings a number of advantages including a fuller, more natural earlobe. This no doubt results from use of tissues that were genetically intended to form the earlobe before the formation of the cleft deformity.

The strength of this paper is its problem-solving approach—using numerous photographs—which highlights the clinical problem and why the recommended technique is logical, although the description of the surgical technique through intraoperative photographs could have been a little more elaborate. The authors have used a methodical and data-driven approach to refine the surgical technique but also to describe their outcome data in terms of both scarring and overall aesthetic outcome. Overall, this new technique shows promise, and the authors are to be congratulated.
